Apply for the Health Sciences Service Program
The Health Sciences Service Program application open annually October 1. The deadline to apply each year is March 1.
Awards are made in spring after the application closes.
Application Submission Requirement
All applicants must submit the following during the application process:
- 2025-2026 Health Sciences Service Program Application (online)
- Two Recommendation forms (sent to applicant after an application is submitted)
- Certification of Educational Debt Form (sent to applicant after an application is submitted)
After applying for the Health Sciences Service Program, you will sent emails to complete the Recommendation forms and Certification of Educational Debt Form.
